The Sunshine Coast Private Hospital’s Cooinda Mental Health Service provides a comprehensive range of mental health services including a 'Transition to Wellness' day program.
About the program
This program enables patients to receive treatment while remaining in their own home. For some, it may even be an alternative to a hospital admission. The program helps you to build a ‘toolkit’ of techniques and strategies to improve functioning; to increase self awareness; and to help you find your path towards ongoing mental wellness.
While participating in the Transition to Wellness program individuals will be supported to set goals and develop relapse prevention and discharge plans. The program includes life skills, goal setting, exercise physiology, psycho education, communication skills, stress management, mindfulness, healthy sleep, skills training (relaxation, medication management), anger management, recreation skills, vocational preparation and self discovery.
The program involves meeting with a small group of individuals (maximum 12) who share similar experiences. This occurs on set days per week (9am – 3pm). The program is facilitated by mental health clinicians who specialise in this area. In addition to the program, individual therapy is available; your Psychiatrist can refer you to a Psychologist to work through issues or challenges you may be facing with your recovery. A course of individual therapy may include up to ten weekly (or bi-weekly) treatment sessions that typically last 60 minutes.
Who should attend?
- Those without adequate support at home
- Those needing early intervention to prevent relapse
- Those able to manage their treatment and provide self care
- Those able to cope in their usual environment
